Understanding Breast Augmentation Surgery
There are many options to consider before your breast enlargement surgery. As a skilled and experienced plastic surgeon, Dr. Lepore is able to make recommendations and guide you through the decision making process. This process is about YOU, so Dr. Lepore will be sure to thoroughly explain his recommendations to keep you informed and comfortable. Together, you will review your options for surgical technique and breast implants at his San Jose practice including:
- Implant type can be either silicone gel or saline filled. Each type of implant has its advantages, and Dr. Lepore will explain these to you so that you can decide on your best option.
- Implant "profile" or "projection" determines how far your breasts will project from your chest. Implants may have a high, medium or low profile.
- Implant shape may either be round or teardrop shaped.
- Implant position can be over or under the pectoral (chest) muscle.
- Incision location can be along the breast crease, around the areola, or in the armpit.

What to Expect
Depending on the scope of your breast augmentation, Dr. Lepore may perform your surgery while you are either under general anesthesia or local anesthesia with sedation. Either way, he discusses his plans with you well before your surgery date.
During your consultation with Dr. Lepore, you will make a number of choices, among them, the placement of the implant, and how it will be inserted. Most patients at our plastic surgery practice in San Jose can choose 1 of 3 incisions that best suits their goals:
- The inframammary incision is made along the crease below the breast, and the resulting scar stays hidden beneath clothing and swimwear.
- The periareolar incision traces around the lower edge of the areola. Once healed, the resulting scar blends with the natural border of the areola.
- The transaxillary incision is made in the armpit. Although this incision is appealing because it doesn't leave any scar on the breast, it's usually more appropriate for women who have chosen saline implants.
Through the incision, Dr. Lepore creates a pocket within the breast and inserts the implant. This pocket can either be over the chest muscle (called "subglandular" because it's beneath the breast gland) or under it (called "submuscular"). Both placement options have their own advantages and disadvantages. In general, a submuscular placement is ideal for women who have very little natural breast tissue because the additional mass of muscle provides more coverage of the implant. However, the best option for you depends on several factors, and Dr. Lepore helps you decide during your consultation.
Once he has made the pocket, Dr. Lepore adds the implant. Silicone implants are inserted full, and saline implants are inserted empty and filled once they are in place. Once both implants have been placed, he evaluates the breasts for symmetry and closes the incision before you're moved to a separate recovery area.
After your breast augmentation in San Jose, CA, you will need to rest for the first 24 to 48 hours. Your breasts will be swollen and may feel tender or sore. After a period of limited activity that typically lasts several days, Dr. Lepore will clear you to resume your normal activities. He will also provide you with detailed guidelines to help you recover properly after surgery. These guidelines offer short- and long-term instructions, such as sleeping on your back for several weeks.
